Did not attend - 'DNA'

On average, 200 booked appointments are wasted each month because patients do not attend.

If you miss 3 or more appointments in a given period, we will notify you that further missed appointments will result in your removal from our list and you having to find another GP.

We have introduced an SMS Text Reminder service to remind patients of their appointments.

We will be sending text confirmation and reminder messages to patients who have an appointment at the surgery with a doctor or nurse. If you do not wish to receive text messages from the surgery please let reception know. If you are happy for us to send you text messages please let us have your correct mobile telephone number.

If you cannot attend an appointment please telephone our reception team on 01865 761651 to cancel the appointment so someone else can use it.

Registering with the Practice

registrationIn order to receive care from a surgery when you want it (rather than in an emergency) you must register with a Practice. To register with us please ask reception for our registration forms. The receptionist will hand you two forms: Family doctor services registration form (GMS1Form), and a Patient Questionnaire

You will need to produce proof of your current address e.g. telephone, gas or electricity bill addressed to you, and some form of photo ID e.g. driving licence or passport. Simply complete the forms and return them to reception and we will arrange for a registration appointment.

Non-UK residents/students studying in the UK for less than 3 months

Non-UK residents or students studying in the UK for less than 3 months are welcome to register as a temporary resident and book an appointment with our doctors and nurses. A fee will apply if you are not entitled to NHS treatment. Please ensure that you bring your European Health Insurance Card (EHIC) with you to each appointment.   

Please specify to reception if you wish to register as a temporary resident as the forms you need to complete are slightly different. Once you have registered with us, Thames Valley Primary Care Agency will send us your medical records from your previous GP. Children under 16 need to be registered by their parents. 


Change of Address / Contact Details

Please inform us if you change your name, address or home/mobile telephone number. Email addresses are increasingly useful for communication from us to you. 

Please note that all changes to date of birth/first name/surname have to be proved by either photocopies of passports, birth certificates or marriage certificates or other official documents, which we have to send to Thames Valley Primary Care Agency. These changes cannot be accepted without this proof.
